In the event that we encounter difficulties verifying your insurance information, if you are uninsured, or if your insurance plan is not covered by us, it will be necessary for you to settle the full payment for your visit prior to your departure from our premises. To confirm our participation in your insurance plan, we kindly advise you to review our Participating Insurance List. Moreover, rest assured that we will undertake insurance verification upon your arrival.

If you find it necessary to reschedule or modify your appointment, we kindly request that you notify us no less than 24 hours ahead of time. This consideration ensures we can accommodate another patient in need of care.

What is the difference between in-network and out of network?

– In-network refers to insurance plans with which our practice has a contract. This means that we have agreed to provide services at a discounted rate to members of these plans.
– Out-of network refers to insurance plans with which we do not have a contract. This means that we are not bound by any discounted rates, and you may be responsible for paying the full cost of your care.

How do I know if my insurance is in-network?

You can call your insurance company’s customer service line to find out if our practice is in your network.

Can I be seen if my insurance is not in-network or if I do not have insurance?

Yes, you can still be seen by our practice even if your insurance is not in-network or if you do not have insurance.
You will be considered a private pay patient, and you will be responsible for paying the full cost of your care at the time of service.

I am a private pay patient, how much will I have to pay?

The cost of your care will vary depending on the services you receive.
Please review our fee schedule below for a detailed list of direct pay / cash fee costs.

Do I need a referral to be seen at your office?

Most insurance plans do not require a referral to see a dermatologist.
However, some specific plans, such as HMOs, may require a referral.
If you are unsure whether or not you need a referral, please contact your insurance company.

How do I obtain a referral?

If you need a referral, you will need to contact your primary care physician.
Your primary care physician will submit a referral authorization request on your behalf to your insurance company.
Please allow enough time for the completion of the referral process prior to making your appointment.

My insurance is in-network, how much will I have to pay?

The amount you will have to pay will depend on your specific insurance plan.
However, you can expect to pay a copay for your evaluation and management service.
You may also be responsible for paying a coinsurance or deductible for any additional services or procedures you receive.

I paid my copay at the time of my appointment, why do I have a balance on my account?

Your copay may only apply to the evaluation and management portion of your visit.
Any procedures or treatments performed during your visit may be billed separately.
Please review your explanation of benefits (EOB) from your insurance company to see what services were covered and what your out-of-pocket responsibility is.

How do I know my deductible and coinsurance amounts?

You can contact your insurance company to find out your deductible and coinsurance amounts.
This information will also be listed on your insurance card.

What are copays, coinsurance, and deductibles?

A copay is a fixed amount you pay for a covered health care service.
Coinsurance is a percentage of the cost of a covered health care service that you are responsible for paying.
A deductible is the amount you must pay out of pocket before your insurance plan starts to pay for covered services.

What are Out-of-pocket maximum/limit

The out-of-pocket maximum is the most you will have to pay for covered services in a plan year.
After you reach your out-of-pocket maximum, your insurance plan will pay 100% of the costs of covered benefits.
